Most teams assume: aligned model + our own documents = a safe system.
Bloomberg researchers tested that assumption (An, Zhang & Dredze, NAACL 2025). They ran 11 LLMs on 5,592 harmful questions, with and without retrieval.
Most models became significantly less safe with RAG:
→ Llama-3-8B: 0.3% unsafe without retrieval, 7.9% with safe documents only
→ GPT-4o: 3.6% → 11.4%
→ Claude-3.5-Sonnet: 0.3% → 0.7%, so the effect depends heavily on the model
𝐓𝐡𝐞 𝐬𝐮𝐫𝐩𝐫𝐢𝐬𝐢𝐧𝐠 𝐩𝐚𝐫𝐭: for Llama-3-8B, 81.8% of unsafe answers came from safe documents. Context changed the behavior, not the content.
Here is how to protect a RAG system:
🔹 𝐌𝐞𝐚𝐬𝐮𝐫𝐞 𝐭𝐡𝐞 𝐬𝐚𝐟𝐞𝐭𝐲 𝐝𝐞𝐥𝐭𝐚: same model, same prompts, with and without retrieval, per risk category, on your own corpus.
🔹 𝐆𝐮𝐚𝐫𝐝 𝐞𝐯𝐞𝐫𝐲 𝐬𝐭𝐚𝐠𝐞: classify the query before retrieval, flag unsafe documents at ingestion, and check the final answer with an output guard.
🔹 𝐔𝐬𝐞 𝐜𝐨𝐧𝐭𝐞𝐱𝐭 𝐬𝐢𝐳𝐞 𝐚𝐬 𝐚 𝐤𝐧𝐨𝐛: the study found more documents meant more risk, so retrieve fewer for sensitive topics.
Best of all? Each layer is cheap, and together they turn “the vendor says it’s safe” into a number you measured yourself.
If your safety testing never switched retrieval on, you haven’t tested the system you ship.

That's from METR's 2025 trial: 16 experienced open-source developers, 246 real tasks from their own repos. Afterwards, they still believed AI had saved them about 20%. Newer tools may do better. The gap between feeling fast and being fast is the part worth measuring.
I made a 2.5-minute video on what held up in four years of AI hype and what didn't.
What held up is narrow: quality checks on a production line, document reading, ranking. In 2016, Geoffrey Hinton said we should stop training radiologists. Mayo Clinic now has over 400, up 55% since then.
What got fragile is long and open-ended. Gartner expects over 40% of agent projects to be cancelled by the end of 2027. One reason is arithmetic: if a step is right 95 times in 100, ten steps in a row work only 60% of the time (0.95^10 = 0.5987, if errors are independent).
Two limits. METR's sample was small, and MIT's 95% failed-pilots figure comes from interviews and a survey. I treat both as signals, not verdicts.
Which have you seen in your own work: a tool that felt fast and measured slow, or a demo that never survived production?
